Forms cannot be processed without both signature and date. Depending on the products and services you would like to use in your account, you may need to submit one or more additional form(s).    Step 2 – Prepare Required Supporting Documents Submit copies of following documents along with your account application forms:   A clear photocopy of the personal information section (with photo) and signature page of a current passport for each applicant.
